Greater Manchester’s Health and Social Care Partnership’s Population Health Transformation Team have a hugely important job in developing the Population Health Plan with many organisations and partners across Greater Manchester, amongst many other things.
Population Health is complex – with many factors contributing to health and well being – such as employment, income, housing, leisure, and lifestyle to name but a few.
This means that system leadership and team working needs to be strong and that staff are supported and developed, and Donna Bradshaw and the Prospect coaches will be working with the team, using real programmes and projects to illuminate the leadership and teamwork learning.
